How Do I Switch from My Previous Dental Office to Nottingham Dental?

When switching dental offices, we’ll need a few details:

  • When was your last appointment?
  • When were your most recent x-rays taken?
  • Which office did you visit?

With your permission, we can request that your current x-rays be transferred, which will require you to sign a consent form.

Additionally, we’ll need your name, address, date of birth, phone number, and insurance information to set up your file and schedule your appointment.

What Is Your Cancellation and Booking Policy?

Our dentists and hygienists reserve valuable time for your appointment. Please provide us at least two full business days' notice if you need to reschedule or cancel, as failure to do so may result in a fee.

What Forms of Payment Do You Accept at Your Office?
